Employee's fingertip amputated in bin dumper

At 2:30 a.m. on May 13, 1996, Employee #1, of Sunsweet Growers, Inc., was routinely dumping fruit at the prune juice prep area. He had used a forklift to set a large, 2,200 lb bin of prunes on the dumper platform, but when he attempted to operate the lower air valve control from his forklift, the bin dumper did not move. He then went up on the elevated operator platform and tried the upper control. When that didn't work, he reached out, grabbed the bin dump frame, and pulled on it. Employee #1 later stated that the bin began to dump immediately without excessive pulling. While it was dumping, the bin suddenly tipped against the frame, pinching his right middle and ring fingers. The tip of one of his fingers was amputated. Employee #1 pulled his fingers free and had a coworker to call 911. The accident resulted from the north bin dumper machine not functioning correctly.
