Employee's finger amputated in meat grinder

At 1:45 p.m. on April 6, 1996, Employee #1, who worked in the meat department at Dollard's Sierra Market, was grinding beef to make hamburger. She was using a small Toledo Scale gravity-feed food grinder that had a 3 in. diameter by 4 1/2 in. long throat above a 2 1/2 in. screw auger. The base of the throat had a 2 1/2 in. by 2 1/4 in. hole formed when an employee put the metallic food pusher in upside down and the small end hit the screw auger, rebounded, and knocked out a hole. Employee #1 was aware of the hole and had been told to use a small meat hook to pull out any meat that was exiting from it. In her haste, she forgot and pushed the meat back into the hole with her finger. Her finger struck against the screw auger and was amputated. According to the store manager, the hole was made 18 months prior to the accident. The manager did all the meat cutting and occasionally other employees ground the meat.
