Employee's fingers injured in chain and sprocket drive

Employee #1, normally a grader at a sawmill, was doing overtime clean-up work. She had started cleaning in front of the roll case that sent lumber from the resaw to the green chain and was working her way under the roll case. The sawmill, which had been on break, started back up while she was cleaning under the roll case. Employee #1 continued to clean when her left hand became caught in the chain and sprocket drive. She was wearing rubber gloves and was able to pull her left hand out of the drive. Her little finger had been amputated at the second joint and her ring finger sustained lacerations. The cause of the accident was working under the roll case while the machine was in operation. The roll case should have been shut down and locked out.
