Employee injured in fall from ladder

On December 12, 1994, Employee #1, a maintenance worker for Farmers Rice Coop in West Sacramento, CA, was climbing a metal portable ladder to patch a hole in a screw conveyor trough. The ladder was leaning against a conveyor 5 ft below that ran perpendicular to the conveyor above. The ladder support point against the conveyor was 8 ft above the floor. When Employee #1 reached a point 5 to 6 ft above the floor, his weight above the support point caused the ladder to pivot. The feet of the ladder slid out, causing Employee #1 to fall to the ground. The ladder was not secured or held, as is required by federal and state standards. First aid was provided by the West Sacramento Fire Department. Employee #1 was transported by ambulance to Sutter Memorial Hospital, Sacramento, CA, where he was treated and released for a broken elbow. He later returned to the hospital to have his elbow set surgically.
