Event: 11/21/1994
Employee dies after fall through roof opening

At about 11:30 a.m. on November 21, 1994, Employee #1 of Sierra West Construction was part of a crew that was remodeling and upgrading a panelized roof structure. They were adding larger beams and columns to hold environmental control units. After waiting for 2 to 3 hours for the cold and wet roof conditions to improve, the crew began replacing the panelized roof sections that had been removed. The glu-lam beam and roof joist had been placed. A framed roof opening had been left open for the required duct work. While moving a 4 by 8 ft sheet of plywood, Employee #1 stepped through the opening. He fell 25 ft to the concrete floor. The employees working on the roof at the leading edge were not using any protective system, such as harnesses and lanyards. First aid was given at the site and Employee #1 was then transported to UC Medical Center in Sacramento, CA. Employee #1 died from multiple internal injuries on December 9, 1994.
