Employee's finger tip cut off in printing press

On December 20, 1994, Employee #1 started up a Hamada 660 two-color printing press. He was dabbing the water form roller when the middle finger of his right hand got caught between the water form roller and the plate cylinder. He shut off the press and backed his finger out. The tip of his finger was amputated and he noticed that the safety bar that sits between the water form roller and the plate cylinder had been removed. No one will tell who removed the bar or when. Employee #1 had operated the machine many times before. The owner and a coworker had also run it.
