Employee's foot amputated after tank rolled over it

At approximately 5:20 p.m. on March 19, 2002, Employee #1 of National Technical System instructed a forklift operator to lift a metal tank of 41 in., width by 41 in., depth by 75 in., height size with casters using a forklift to place the tank on a concrete block of 40 in., width by 42 in., depth by 23.5 in., height size. When the tank was placed on the block, it started to roll off the block to where Employee #1 was standing. Employee #1 tried to stop the tank from falling off, but the tank was too heavy for him. He then attempted to move away to avoid being hit by the tank, but the falling tank landed on Employee #1's right foot and crushed the front half. Employee #1 was hospitalized for amputation.
