Employees'S Finger Amputated By Unguarded Rotating Blades

At approximately 4:00 p.m. on June 9, 2002, Employee #1 amputated all four fingers of his left hand when they were caught in the unguarded rotating blades of a feed port. There were no identification numbers on the feed port. The feed port, an elevated hopper that feeds plastic pellets to extruder machines below, was located on a mezzanine above the work area. The rotating blades were located 8 inches inside the unguarded feed port.
