Investigation Summary
Employee #1, a seamstress, was operating a fusing machine when she noticed a piece of masking tape on a metal roller that drove a large, flat, rubber belt. She asked a coworker to help her lift the heavy metal cover of the machine, then reached in to remove the piece of tape. The roller caught the sleeve of her blouse and pulled in her hand, crushing it.
