Investigation Summary
Employee #1, a laborer, was sweeping cardboard scraps into an 8 in. diameter floor-level opening that was 18 in. above conveyor belts that carried the scraps to a baling hopper. The scraps became jammed and Employee #1 reached into the opening to clear it. The belts caught his glove and pulled in his hand and arm. He suffered a fractured his lower right arm and wrist.
