Investigation Summary
Employee #1, a foreman, was installing piping for a fire sprinkler system in a building under construction. He was working from an elevated work platform extended to its maximum height of 11 ft. When Employee #1 attempted to travel with the platform at this height, a platform wheel went into a floor opening that was 2 ft square and 4 1/2 in. deep. The platform overturned and Employee #1 struck the concrete floor. He suffered lacerations and fractures to both his legs and ankles, skull, and nose.
