Employee injures head in fall through skylight

Employee #1 was sent to caulk a water leak along the edge of a 36 in. wide by 16 ft long domed skylight with four sections of 1/8 in. thick plexiglass with a 9 in. high curved dome. The curve of the plexiglass met the concrete wall. The skylight was located between two vertical walls, leaving no room to walk. Employee #1 apparently had been trying to seal the area where the plexiglass meets the walls and had both his feet in that area. His foot may have gotten stuck, and as he tried to move forward, he lost his balance and fell head first onto the last section of the plexiglass. The plexiglass broke and Employee #1 fell 11 to 12 feet to the concrete floor below. He sustained serious head injuries and was hospitalized three days.
