Employee's finger amputated by crimping machine

Employee #1, a production machine operator, was operating a crimping machine that attaches small terminals to two electrical wires integral to a plastic vacuum hose and crimps the terminals to the wires by power press action. Although the machine's point of operation was guarded, a small one inch opening around the side of the guard was not protected from the action of the press. Employee #1 reached around the guard and placed her right middle finger into the opening. It was amputated at the first knuckle.
