Event: 10/07/1993
Employee places head in press, kills self

Employee #1 placed his head under a plastic dome molding press. This machine travels very slowly in an upward and downward motion. Employee #1 held his head in this position until the press made contact. Other employees were in the area but no one was looking at the time of the accident. Another employee stated that he was working with Employee #1 when the accident happened, but he did not hear or see anything. When he turned around he saw Employee #1 leaning across the machine. He rushed over to see what was wrong. He noticed a mark on his hand, then he removed Employee #1 and called for help. Someone called 911. The chairman of the board stated that Employee #1 had tried to shoot himself in the arm with an air-operated nail gun a few days before the accident.
