Investigation Summary
On September 20, 1993, Employee #1 was working in the clean room and ran out of caps. She proceeded to the stock room. On the way to the stock room, Employee #1 entered a hall where the nighttime janitors were cleaning. The janitors had just washed the floor and had put up two signs designating that the area had been washed and was wet and slippery. Employee #1 proceeded through this area and slipped and fell on her knee.
