Employee Fell Inside of A Truck

At approximately 11:30 a.m. on June 27, 2002, Employee #1 and coworkers were applying a slurry seal to the street using a slurry applicator truck. The truck has a large sand bin as the main body shape. Employee #1 was training to be a driver of the applicator truck, and was requested to work in the back of the truck breaking up lumps of sand inside the bin area as the truck drove down the street. The truck came to a winder area of the street, making the first pass, then backing up to begin a second pass. When the truck stopped after backing up Employee #1 was climbing down the ladder on the back of the truck. Employee #1's coworker not knowing the bin was empty signaled the truck driver to begin a second pass. When Employee #1's coworker started the truck, he lost his balance and fell approximately 6 ft onto the back of the truck, striking various parts of the mounted machinery in the truck. Employee #1 was taken to the hospital and treated for several broken ribs and a lacerated liver.
