Investigation Summary
On April 16, 2003, Employee #1, a corn tortilla machine operator, was removing built-up mix from a machine. The employee walked to the opposite side of the machines' working platform to remove some built-up corn tortilla mix. After removing the mix with a push stick, the employee brought his right hand straight down and inadvertently placed his index finger directly between the unguarded moving sprockets. The employee's finger was broken in four places and was unable to be reconstructed. His finger was amputated just above the hand knuckle. Employee #1 was not hospitalized.
