Investigation Summary
On August 8, 2002, Employee #1, an electrician, was working on the chiller of a new HVAC system at a new information technology building at Edwards AFB. He was pulling control wire into the unit when he contacted a 480-volt busbar and was electrocuted. He was pronounced dead at the scene. Employee #1 failed to de-energize and lock out the system before starting to work.
