Employee Fractures Back in Fall from Scissor Lift

On November 23, 2007, Employee #1 was working in the production area of a plant manufacturing large plastic liquid storage tanks. Employee #1 was mounting the lid to the tank mold that was approximately 10 ft above the floor. Employee #1 was installing the bolts in the lid. Employee #1 did not use the platform extension on the Genie Scissors lift and put the lift next to the mold. There are bump outs on the bottom of the mold. The mold was being rotated to put in the next set of bolts when the bump out hit the lift and turned it over, knocking Employee #1 to the ground. As a result, Employee #1 injured his back. Employee #1 was hospitalized.
