Investigation Summary
On July 27, 2007, Employee #1 was monitoring the flow of water bottles into the case packing machine. A bottle fell over and was going to disrupt the flow of the product into the case. Employee #1 did not disengage the machine and reached into the machine to upright the bottle. The case pusher bar indexed. This bar caught Employee #1 right forearm crushing it. Employee #1 was hospitalized.
