Employee slides off plastic tank and fractures hip

On January 13, 2003, an employee of John Taylor Fertilizer Company's Yuba City facility had completed replacing a 1600-gallon poly-tank onto a trailer when he noticed that the lid located on top of the tank was not properly closed. He climbed onto the top of the tank via the trailer's fender, and as he closed the lid, he began to slide off the tank. He was unable to stop his slide and hit the shop floor. The Yuba City Fire Department responded and transported the employee to Memorial Hospital in Marysville. The employee fractured his left hip.
