Investigation Summary
At approximately 3:20 p.m. on November 6, 2002, Employee #1 climbed on a stack of plastic pallets to gain height in stacking material. Employee #1 did not taking into consideration the fact the pallets were plastic and were made more slippery by rain. Employee #1 slipped and fell approximately 4 ft 8 in. and sustained a fractured right hip and wrist. Employee #1 was hospitalized.
