Investigation Summary

Investigation Nr: 201172111
Event: 06/20/2002
Employee injured when arm catches in equipment

At about 8:30 a.m. on June 20, 2002, Employee #1 and other crew members of Teichert Precast were running the concrete grade ring manufacturing machine at Jackson Highway in Sacramento, California. They had a problem with various sensors in the machine. It was difficult to operate the machine in the manual mode when not running automatically. Employee #1 and other crew members were repairing a sensor, which controlled the press head and concrete dump box. When Employee #1 reached to the machine holding the sensor, the other employee, a foreman, hit the start button. When the concrete dump box started moving forward the foreman released the button but it didn't stop. As the concrete dump box reached the end of the rail, it pinned Employee #1's arm against the frame of the machine. First aid was provided at the site before Employee #1 was driven to Sutter General Hospital in Sacramento, California. He was treated for muscle damage to the right bicep. A part of the muscle was removed.
