Investigation Summary

Investigation Nr: 201103843
Event: 02/11/2003
Employee injured after a beam tips over on his leg

At approximately 10:00 a.m. on February 11, 2003, Employee #1, of Dura-Built Construction Company, was insetting hangar bolts into a glu-lam beam that had been placed on the slab in a standing position of a new construction building. A coworker, operating a scissor lift, was in a stopped position nearby. As this coworker turned to talk to another coworker at the site, the joy stick control was bumped. The unit moved a bit, striking the other beam sitting on the ground near Employee #1. As this second beam was struck off center, it became skewed, and tipped over and onto the back legs of Employee #1, who was in a sitting position. Employee #1 sustained leg fractures for which hospitalization was required.
