Investigation Summary

On September 13, 2002, Employee #1, a California State Prison inmate assigned to a work camp run by the California Department of Forestry and Fire Protection, was standing on the platform of a tube and coupler metal scaffold, polishing the sides of a truck trailer. He was using a two-handed Milwaukee 7-in. automotive buffer when he stepped too close to the edge of the work platform. Employee #1's foot became caught in the 4.375-in. wide opening between the platform and the frame and he fell off the scaffold, which then upended. He sustained a fractured tibia and fibula just below the knee, for which he was hospitalized. The scaffold was on level ground and the wheels were locked.
