Investigation Summary
At about 6:45 a.m. on June 12, 1998, a top lift operator was replacing a flex hydraulic line on the left side of a Taylor top lift on a 27-ft elevated 4-ft by 12-ft platform raised by a forklift on Jeffrey Road in El Centro, CA. While working, the employee struck his head against the left front fender of the top lift and his feet and body struck the rotating left tire, causing him to fall to the ground and killing him.
