Investigation Summary
At 11:20 a.m. on December 17, 1994, Employee #1, of McKay Roofing Co., Inc., along with three coworkers and a foreman, were replacing cracked or broken rafters on a commercial building. A plywood roof panel and a skylight cover had been removed to expose the 2 by 4 rafters that were to be repaired. When Employee #1 put his foot on a 2 by 4 to brace himself, it gave way, causing him to fall 15 ft through the skylight opening and land on his head and neck on a concrete floor. Employee #1 was flown to Palomar Hospital, Escondido, CA, and admitted to critical care with multiple compound fractures of the skull. He died on December 28, 1994, at 2:00 p.m.
