Investigation Summary
Employee #1, of Direct Labor Contracting, was using a Delta 10 in. table saw to cut a 4 ft by 8 in. by 3/8 in. piece of wood flooring. He was standing in front of the saw when the board hung up on the blade, then slipped, causing his left index finger to contact the blade. The finger bone was cut 3/4 of the way through above the second knuckle. The nerves to the finger were also cut. Employee #1 will suffer some permanent damage. There were no witnesses to the accident. The saw blade guard was in place.
