Employee's finger tip amputated in exhaust fan

Employee #1, a journeyman aircraft mechanic for a company that manufactures airplanes, had been assigned to remove and reinstall a flow sensor that had been installed backward in an avionics cooling system. The flow sensor was a pilot tube mounted in a short section of duct, adjacent to the avionics exhaust fan. Employee #1 had been instructed to wait for electricians to de-energize the cooling system's circuit breakers before starting the job, but he decided he could perform the repairs with the cooling system still on. While he was replacing the clamp between the fan and the sensor, his left hand entered the avionics exhaust area, and a portion of the distal phalanx of his fourth finger was amputated in the fan blades. Employee #1 had not worked on the sensor or exhaust fan before, and was not aware of the proximity of the fan blades to the duct containing the flow sensor.
