Investigation Summary

On September 8, 1994, Employee #1 was working for a contractor at a liquor distributor's facility. He and another employee of the contractor were installing an automatic machine (for bottle labels). Employee #1 was standing on level, dry ground, assembling 1 1/2 inch or 2 inch threaded pipe with a 36 inch pipe wrench. One end of the pipe was held in a Rigid pipe vise. Employee #1 bore down on the wrench and sustained a hernia. He was hospitalized and surgery followed. Employee #1 is a journeyman electrician and had successfully done this same operation numerous times. The injury was not caused by a slip. The tools in use were in good condition.
