Investigation Summary
At approximately 12:15 p.m. on March 7, 1994, Employee #1 and a coworker were involved in a conveyor repair operation to a #100 pan feed conveyor, which was located on the southeast side of the Wood Waste Recovery Department in the outside refuse yard at Falcon Refuse Center, Inc. Angle flights that had been newly installed on the conveyor's chains on March 3 and 4 were broken and causing the conveyor to jam up, and the employees were required to remove the conveyor's belt to replace them. As the work concluded, the lead man went to the shop area to fab up the last flight to complete the repair operation. Employee #1 was cleaning up the area above and below the conveyor and had started to inspect the flights and bearings underneath when the employee at a nearby sorting conveyor accidentally turned on the #100 conveyor. Employee #1 was wedged between a support brace and the moving conveyor flights and died of his injuries.
