Investigation Summary
At about 3:00 p.m. on February 11, 1994, Employee #1 was working on top of a portable oil storage tank. The oil in the tank was increasing in temperature. The tank also had some water contained with the oil. When the temperature reached the boiling point of the solution in the tank, it released hot vapor that engulfed Employee #1. He received 1st and 2nd degree burns to his face and leg for which he was hospitalized.
