Investigation Summary
At approximately 6:20 a.m. on June 13, 1994, Employee #1 was driving a forklift truck in the display department of a warehouse. Employee #1 exited the fork lift to get a roll of tape from a shelf and the forklift continued to roll. Employee #1 was caught between the forklift and the shelving and received a broken rib and hip for which he was hospitalized.
