Investigation Summary
At approximately 12:45 p.m. on November 24, 1994, Employee #1 was standing on a counter, securing an electrical cord approximately 7.5 feet high. The counter suddenly moved because it was not anchored to the floor. Employee #1 lost his balance and fell. He suffered fractures to his right distal radius and ulna.
