Investigation Summary
At approximately 10:30 p.m. on September 14, 1999, Employee #1, a material handler employed by Bomatic, Inc., was standing on a large cardboard box next to a large roll-off trash container. He was shoving scrap cardboard materials from a pallet elevated by a forklift into the dumpster when he lost his balance and fell approximately 49 in. Employee #1 sustained a concussion and was transported by private vehicle to the Chino Medical Clinic. He was then transferred to St. Bernardine Hospital, where he was treated for four days.
