Employee Sustains Multiple Fractures in Fall

At approximately 9:30 a.m. on October 08, 1999, two employees were repairing and welding a detached 30-in. bell form (a reducer ring 30-in. inner diameter and 36-in. outer diameter) of a concrete pipe forming machine (Packerhead Number 3, Model Number PH48), which did not fit properly in the Bell Form Ring Assembly after initial welding because of burrs. During welding, Employee #1 was working within 24" of an unguarded (60-in. diameter) floor opening, stepped back and fell 8-ft 4-in. to the bottom of the turn table base. Employee #1 fractured his lower back, broke three ribs and suffered head injuries. Employee #1 was hospitalized for four days.
