Investigation Summary
At approximately 11:10 a.m. on April 30, 2007, Employee #1, along with his foreman, placed a 28 ft portable aluminum extension ladder, a Werner 300 pound Capacity, through a roof hatch opening to access the roof in order to install HVAC plenums. The roof hatch opening was over 20 ft above the concrete floor. Another coworker climbed onto the roof first and held the top of the ladder while Employee #1 followed. When Employee #1 was a few feet from the roof, the coworker let go of the ladder and walked away. The ladder slid from under Employee #1, and he started to fall. He attempted to grab on to the roof but was unable. He fell approximately 16 ft fracturing his ankle and was hospitalized.
