Employee's hand fractured when caught in machinery

At 7:40 p.m. on November 3, 1995, Employee #1, of G.S. Roofing, a manufacturer of roofing shingles, was operating an Architect 80 laminating/cutting machine while standing on its right side. While the machine was running, Employee #1 saw a piece of shingle that was jammed on the roller. When he reached in to clear out the shingle he became caught in the nip point of the rollers. He was taken to Harbor General Hospital and hospitalized for a fractured right hand and various other injuries. Causal factors in this accident include the fact that Employee #1 did not de-energize the machine before clearing out a jam and did not use a tool to remove the piece of shingle. In addition, the nip point of the machine was not guarded.
