Investigation Summary
Around 9:00 a.m. on October 9, 1995, Employee #1 and a coworker were modifying a special /new size wooden skid base. The coworker was using a forklift truck to keep the skid base inclined to the ground by supporting it between the ground and the forks of the truck. After removing the bolts holding the 4 in. by 4 in. lumber, Employee #1 attempted to dislodge the 4 by 4 from the skid using a sledgehammer. His sledgehammer stroke missed the target and his left index finger struck against the adjoining 2 by 4, shearing off his fingertip, which was found next to the 2 by 4. Employee #1 was treated in the company's medical unit and then taken to the Long Beach Memorial Medical Center where he was hospitalized. The skid base should have been placed horizontally on the ground before being hit by a sledgehammer. In addition, the requisite training for Employee #1 and the coworker was incomplete and ineffective from a safety standpoint.
