Investigation Summary
At approximately 10:40 a.m. on January 24, 1997, Employee #1, a welder, was at a height of approximately 15 ft, installing a 10 ft pipe onto the boiler plant pipe rack. He was climbing onto the scaffold to reposition the pipe when he slipped and fell onto the concrete floor. Employee #1 suffered head contusions and fractures to his left knee. He was hospitalized as a trauma patient.
