Employee's finger amputated by table saw

On September 14, 1995, Employee #1 and a coworker were told by their supervisor to start a new job sawing two flange widths of a T-section. The job was set up in a table saw in the quick response department. The saw guard that would have given some protection was not used. Because the job had a tendency to lift up from the saw due to vibrations and the lack of vertical force to keep it in contact with the saw, it needed to be pressed down by hand with frequent grasp/regrasp at the discharge end of the saw. Employee #1 brought his hand too close to the saw and his finger was amputated. Employee #1 was treated at the Western Hand Center. The accident's causal factors are as follows: failure to use appropriate guard/other protective features; failure to use appropriate machine with appropriate equipment fixture; and failure to require training.
