Investigation Summary

Investigation Nr: 171005952
Event: 08/31/1995
Employee's finger amputated in brush machine

On August 31, 1995, Employee #1 was operating an employer-designed wire brush machine in a cutting department. He was standing in front of the machine as he fed 8 in. metal square tubes into the machine for end finishing. Normally, the conveyor chains on the machine pick up one piece of tubing at a time. Due to misfeeding, the conveyor picked up two pieces. To avoid jamming the machine, Employee #1 attempted to pick up the extra piece with his gloved hand without stopping the machine. The conveyor pulled his hand inward and his ring finger was caught between the conveyor and the top guide. The finger was amputated at the first digit.
