Investigation Summary
At approximately 6:15 p.m. on August 22, 1994, Employee #1 was cleaning a tanker truck. He lowered a drop light into the tank and the light bulb broke, creating a flash fire. Employee #1 suffered first- and second-degree burns to 30 percent of his upper body and required hospitalization. The light was not approved for the tank atmosphere and the bulb was not guarded.
