Investigation Summary
Employee #1, a shifter on a crew of six, was assigned to help relocate a ventilation fan. At 10:00 p.m., they had gone underground at the north access shaft and had ridden the man-trip to the track level room. Between 10:30 and 11:00 p.m., the walking boss had lined out the crew, and at 11:00 p.m. they had walked to the ventilation fan to survey the task, arriving there at 11:40 p.m. At 12 midnight, Employee #1 and three coworkers went to the La Brea shaft to get two 6 ton chain falls Their plan was to use the chain falls to hoist and lower the ventilation fan to the deck, then to use come-alongs to skid the fan onto an electric sled. At 1:00 a.m., the four men were returning with the chain falls when the walker told them there had been a change in plan. The plan now was to create a makeshift hoist with a 1/2 in. wire rope. At 2:00 a.m., the crew had fashioned the hoist using a 40 to 50 ft long wire rope running at an angle from the electric sled near the deck at the front of the fan around to the back of the fan. At 3:00 a.m., the ventilation fan was being hoisted down to be slid on the cable toward the electric sled. At 3:30 a.m., the front of the fan was on the deck but had become stuck at the electric sled. At 3:45 a.m., coworkers were prying up the front of the ventilation fan while Employee #1 was pulling down at the rear. The rigging failed at the electric sled and the ventilation fan, pinning Employee #1. He suffered a fractured left femur, tibia, and fibula, and a severed artery. At 4:00 a.m., emergency services was called and Employee #1 was transported to the hospital.
