Electrician Strikes the Back of Head in a Fall and Dies

On December 3, 2001, Employee #1, an electrician, with H. Lima Company, was working with a coworker at a site where gypsum is crushed and milled for further sale. Employee #1 was standing on a level concrete floor that bordered a level asphalt paved yard surface, had been making repairs to a Grove, SM2632E, Manlift. It is a battery powered, scissor lift type, personnel elevating platform. Employee #1 struck the back of his head during a fall on the same level. Employee #1 was found by the work site Foreman lying on his back at first unable to speak, then followed by incoherent speech and then combative reactions to assistance by emergency medical personnel. At some point during or following emergency medical transport, Employee #1 lapsed into a comatose state. The reported injuries included a skull fracture, subdural hematoma, and extensive intracranial hemorrhaging. When Employee #1 regained consciousness he had no memory of the incident related to the injury and there were no eyewitnesses to the occurrence. No evidence is found to establish the specific cause of the accident.
