Employee's feet crushed when caught under forklift

Employee #1 was on her lunch break and was walking to an alternate lunch area on the south side of the plant as an empty forklift was going through vinyl curtains as it left the cold storage area. The forklift's horn sounded but Employee #1 apparently was not paying attention. She was struck by the forklift and knocked down. Her feet went under the steer wheels and became caught under the vehicle. She sustained crushing injuries to both feet and the toes on her right foot had to be amputated. Employee #1 was on the wrong side of the pedestrian walkway at the time of the accident. The vinyl curtains were also dirty, limiting the view of the forklift driver.
