Employee crushed in forklift overturn

Employee #1 was operating a Toyota forklift to load raisin bins onto a flatbed truck. As he finished unloading the forklift, he would back up and turn to the left to go for more bins. According to witnesses' statements, Employee #1 was moving too fast when he turned the wheel sharply, causing the forklift to become unbalanced. It toppled over and trapped and crushed Employee #1. He was not wearing a seat belt.
