Investigation Summary
EMPLOYEE #1, A CLEANUP PERSON, WAS ASSIGNED TO CLEAN UP COTTON DEBRIS IN THE BASEMENT BELOW THE COTTON PRESS AND ITS SURROUNDING AREA. WHILE THE CLEANING AREA AND ADJOINING EQUIPMENT WAS IN OPERATION, EMPLOYEE #1 OBSERVED AN APPROXIMATELY 1-INCH BUILDUP OF COTTON LINT ON THE COTTON PRESS. HE WAS UNABLE TO BLOW THE LINT OFF WITH AN AIR HOSE BECAUSE THE LINT WAS BEING CAUGHT BY HYDRAULIC OIL STUCK ON THE MACHINE. EMPLOYEE #1 REACHED INTO THE AREA BELOW THE FOLLOW-BLOCK, WHICH WAS UP AT THE TIME, TO REMOVE LINT BUILDUP. THE PRESS CYCLED AND THE FOLLOW-BLOCK CAME DOWN, CRUSHING AND FRACTURING EMPLOYEE #1'S LEFT HAND.
