Employee injured when struck by falling pallet of material

Employee #1 was cleaning up a product spill in a warehouse. The material had fallen off a storage rack, and while Employee #1 was cleaning, another pallet stack fell on her. She sustained injuries that required hospitalization. The first spill may have weakened or damaged the product cartons, causing the second spill. The cause of the spill could not be determined because the materials had been disposed of after the accident. According to witness statements and the condition of the warehouse at time of inspection, the employer had not changed its storage process since the accident.
