Employee injured in fall when plywood sheet gives away

At 8:45 a.m. on August 16, 1995, Employee #1 and two coworkers were installing 3/4 in. tongue- and-grove 4 by 8 plywood sheeting across 36 ft long trusses that were on 48 in. centers. Employee #1 was using a 2 in. by 4 in. by 5 ft long board against the leading edge and then banging the sheets in place with a mallet. The ends shared the approximately 4 in. wide top of the truss, and the center of the sheet was supported by a similar truss. The crew had experienced problems aligning the sheets on the trusses. On returning from a brief water break, Employee #1 walked onto the eighth sheet of the seventh course from the east end. He was crossing over the midpoint when the west end of the sheet went down. Employee #1 fell to the floor, striking his head and face on the work basket of a parked Zoom Boom. He suffered fractures to head and face and severe lacerations to his face, nose, and mouth. Employee #1 was transported to Tulare District Hospital, where he was treated and released.
